The provisional entitlement

Once you have passed the medical, you need send the results of your medical (D4 form) along with an application form called a D2 to the DVLA for a provisional PCV entitlement to be added to your existing licence.

You can apply to the DVLA for a D2 provisional entitlement application Pack online. Complete the form with your name and address and tick the box D2 Pack - Application for a large Goods Vehicle (LGV) or Passenger Carrying Vehicle (PCV) licence. In a few days the DVLA will send you an application pack.

If you have the old style paper licence issued before December 1999 you will also need to make an application for the newer style photo card licence. Although any licence entitlements may last for longer, these individual photo cards will have to be renewed every 10 years thereafter.
